Herbs
Herbal formulae are prescribed when appropriate and most are available
in our office.
Most formulae are between $8 and $15 per bottle.
For refills of your existing formula(e), please call first to ensure we
them in stock.
Forms
A complete set of forms is available for you to download, print out and fax/email
or bring to
our office in preparation for your first appointment.
Bring your photo ID (Drivers License or Passport), insurance card (if
appropriate) and your
completed forms to your first appointment.

Insurance
We are an out-of-network provider with most insurance companies and remain in-network with a few. To verify coverage, please call the office, supply us with your insurance information and we will contact your insurance company.
Methods of Payment
All four major credit cards accepted, as well as checks and cash.
Payment is expected at the time of arrival for your appointment, whether
co-pay or balance in full.
